Withdrawal Form

The Withdrawal Form can be submitted in two ways. Please choose your option.

1. Visit the Registrar's Office in person, McMullen Hall, 1st floor. 
2. Access  Withdrawal Form as a fillable pdf. This form is NOT ACCEPTED through Docusign. Once completed, save to your computer and email as an attachment to registrar@msubillings.edu.
