Please note that there is a new feature on the forms.  The originating faculty member should type his or her name into the line next to the signature, like this:

New Course Form

Change/Delete Course Form

Modification of Existing Program Form

Deletion of Program Form

New Certificate Forms

New Program Forms

Academic Foundations Course

 

PLEASE DO NOT CHANGE THE SIGNATURE LINES ON THE FORMS
