OPEN ENROLLMENT

                                                   Insurance Workshops

 

The Human Resources Office will be conducting open enrollment workshops to discuss changes to our insurance plans. 

Human Resources Staff will also be available to assist eligible employees in completing their CHOICES enrollment forms.

Workshops will be held on the East Campus and College of Technology.  Please review the dates, times, and

locations on the attached schedule and select a workshop to attend.

CHOICES enrollment packets will be distributed to eligible employees during the workshops.  To safeguard your

confidential information, we will only be distributing your enrollment packet to you. 

CHOICES enrollment packets will not be mailed.

Reminders:

·        Attend any workshop to complete the enrollment form.

·        Spouses and Adult Dependents are invited to attend the workshops.

·        Enrolled family members, 18 or older, must also sign the enrollment form.

Completed enrollment forms must be received in Human Resources Office by

Monday, May 5, 2008, at 4:00 p.m.
