Fingerprint & Background Check Information

Open both links to access fingerprint procedures, information and the self-disclosure form. The self-disclosure form needs to be completed and signed in front of a Notary.  Notaries are available in banking institutions, hospitals, and on campus. The Field Experiences & Licensure office has a staff member who is a Notary. 

The fingerprint handout provides local ( Billings ) information.  If living elsewhere, county sheriff’s office may also provide fingerprinting services. 

When submitting your fingerprint card to the licensure official, remember to attach a check or money order for $29.25 written out to Criminal Justice Information Services Bureau.
