In order to receive assistance according to the terms and conditions detailed in this announcement, you must verify your specific requests for assistance by submitting the "NEW MSU-B - JEP Assistance Request Verification Form" by July 25, 2006. Only those requests for assistance submitted with this form by July 25, 2006 will be applicable to the terms and conditions of the above referenced announcement.
